If you are a current or previous patient, and would like to request your personal medical records, please contact my directly and in writing, via e-mail, with specifications for which information you are requesting. Please allow for five business days for the request to be fulfilled. If you are requesting electronic records, there is no charge. If you require documentation that has to be printed and/or mailed, there is a $2/page charge plus whatever the shipping charges are.
